There are a few prerequisites before beginning the process of getting a card that allows purchasing and use of medical marijuana, that must be met by every patient.
1. The patient must be at least 18 years or older.
2. The patient must be an Illinois resident.
3. The patient must be diagnosed with a debilitating medical condition.
4. The patient must get a recommendation from their primary physician.
The first thing on the checklist is to meet your physician to begin the discussion on using medical cannabis as a treatment for your condition. This is a crucial first step as it will allow you to understand the effects of medical cannabis and get a professional’s advice for deciding if medical cannabis is right for you. Your doctor must complete and mail a Physician Certification form to the Department of Health’s Division of Medical Cannabis. Your appointment has to have been within 90 days of the submitting of your application so make sure you stay in contact with your physician in order to expedite this process.
All patients must fill out an application for a medical card. Fill out the whole application. If you would rather not designate your Caregiver, the dispensing organization that will provide you with access to medical cannabis, then leave that section blank. Click here for the application.
In addition to a filled out patient application form you’ll need to provide two items that qualify as proof of residency. Acceptable forms include: a state ID, a valid voter ID, a drivers license, bank statements and others. All of the requirements are listed in the application.
Patients must include a photograph taken less than a month before submitting their application. Think passport photo. Make sure that you’ve got a plain background without any artistic filters, that you’re facing the lens straight on and that the photo itself is at least 2 x 2 inches or larger. If you can’t take it at home find a local passport photo service.
Send in an attached color copy of your Illinois-issued photo ID. Must be at least 18 years old; if the patient is a minor their parent or legal guardian is required to get recommendation from two physicians.
A fingerprint consent form, along with a receipt from the fingerprinting, must be sent in within 30 days of the fingerprinting and must be included with the patient application. Click here for the form.
All patients must select a city or county from a list provided on the patient application form to designate the location of their caregiver. Once registered the qualifying patient will be asked to specify a medical cannabis dispensing organization and will be required to fill out a separate Caregiver Application form that includes copies of documents necessary in the patient application form. There is a non-refundable $100 fee (payable by check, money order, or credit card if done online) that must be sent in with the patients application. The fee is reduced to $50 for Veterans and patients who are enrolled in the federal Social Security Disability Income (SSDI) or the Supplementary Security Income (SSI) program. Verification must be provided in the form of a copy of a veteran’s DD214, or a qualifying patients copy of a letter or other documentation from the Social Security Administration.
